Virtual Adventures in Wonderland Summer Dance Camp!

June 7 – 11, 2021, Monday – Friday
2:00 pm – 4:00 pm
(K – 2nd Grade) This five day virtual camp will include movement classes, plenty of social and emotional connection time, as well as group check-ins and warm-ups each day to get us started! Here is a sample schedule:
1:45 - 2:00: Log On/Check-in
2:00 - 2:10: Welcome
2:10 - 2:30: Warmup/Get-To-Know-You Games
2:30 - 3:00: Create Your Own Dance Class
3:00 - 3:30: Break/Snack/Craft
3:30 - 3:55: Sampler Dance Class
3:55 - 4:00: Camp Goodbye/Check-out
Note that the price of camp is only $40.00! We are thrilled to be able to continue to offer five days of summer dance camp at this price. Without this scholarship option, the actual cost of camp would be $150.00. In order to keep the Education & Community Engagement Summer Camp at an affordable price for all of our students, we count on donations from individual supporters of Colorado Ballet. If you are able, we ask that you consider making an additional donation toward this programming in the form below. Due to the financial impact of COVID-19, Colorado Ballet is doing our best to support our teaching artists and students on a limited budget. As a nonprofit organization that relies on the support of the community, Colorado Ballet deeply appreciates your generosity.
If you have more than one student at home who would like to participate in these summer camp, please register each student individually and ensure the demographic information is unique to each student registered.
Also consider that, depending on your student's age/virtual program experience, they may need assistance during some or all of summer camp. This especially may apply during craft time or in the event of any technical issues. Please plan accordingly based on your student and their needs.
PLEASE NOTE: The email address provided in this form is where your receipt, live Zoom link, and all summer camp communications will be sent.
Colorado Ballet retains the right to refuse service to any student whose behavior interrupts the goals, progress and general well-being of the camp. This form and permission slip must be filled out, acknowledged, and consented to by the parent/guardian/caretaker for the student to enter and continue with classes in the virtual program. By acknowledging and consenting to the terms within this form, the parent/guardian/caretaker understands the rules and regulations of the program.

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM VIRTUAL DANCE CLASSES
I, PARENT/GUARDIAN OF SAID CHILD(REN) HEREBY ASSUME ALL OF THE RISKS OF MY CHILD(REN) PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THESE VIRTUAL DANCE CLASSES, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective reigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of my child(ren), for my child(ren)’s death, disability, personal injury, or actions of any kind which may hereafter occur to my child(ren) during VIRTUAL DANCE CLASSES BY THE FOLLOWING ENTITIES OR PERSONS: COLORADO BALLET and/or their directors, employees, or volunteers.(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE THE FOLLOWING ENTITIES OR PERSONS: COLORADO BALLET and/or their directors, employees, or volunteers from any and all liabilities or claims made as a result of participation in VIRTUAL DANCE CLASSES, whether caused by the negligence of release or otherwise. I acknowledge that COLORADO BALLET and their directors, employees, and volunteers are NOT responsible for errors, omissions, acts, or failures to act of any party or entity conducting VIRTUAL DANCE CLASSES on their behalf. I acknowledge that this activity may involve injury. The risks include, but are not limited to, those caused by jumping, leaping, turning, stretching, or any dance movement, lack of hydration, improper dance floor, and actions of other people including, but not limited to, participants, and/or producers of the activity. I hereby consent for my child to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during VIRTUAL DANCE CLASSES. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT. I SIGN IT OF MY OWN FREE WILL. I certify that my child(ren) is physically fit, has sufficiently prepared or trained for participation in VIRTUAL DANCE CLASSES, and has not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my child(ren)’s participation in VIRTUAL DANCE CLASSES. I agree to the use of any photographic or video graphic images taken during the session for Colorado Ballet publicity purposes. I acknowledge that this Accident Waiver and Release of Liability Form will be used by COLORADO BALLET , the director, class holders, sponsors, and organizers of the activity in which my child may participate, and that it will govern my child(ren)’s actions and responsibilities at VIRTUAL DANCE CLASSES. I permit my child to participate in VIRTUAL DANCE CLASSES.
